Resumen

The palindromic rheumatism was first described in 1944 by Hench and Rosemberg. It is and entity characterized by multiple recurrent episodes of arthritis and periarthritis (mono or oligoarticular) that may last hours or days, they dissapeared without articular lesions. We present a 69 year old male patient with a history of hypertension, and presumptive diagnosis of gout due to recurrent episodes of arthritis and periarthritis in the last thirty years of at least two joints which lasted a few days, and were self limited. The patient was admitted because of arthritis and periarthritis of both wrists, knees, ankles, elbows and hands. He presented with fever (38-39º C), intense articular pain and anorexia. With a presuntive diagnosis of palindromic rheumatism and the lack of response with non steroideal antiinflamatory drugs, 20 mg of metylprednisolone were initiated with an excelent response. We present the case, and a revision of the literature refered to diagnosis and treatment of what we considered and under diagnosed entity
