Porphyrias are metabolic hem biosynthesis enzymatic disorders. Porphyria cutanea tarda (PCT), one of is varieties, is a hereditary or acquired disease. It can be unleashed by iron overload, alcohol, estrogens and other. In these patients the hepatic compromised can be associated to cirrhosis, iron overload or C and B viral infection, all predispose factors of hepatocellular carcinoma. We present the case of a 69 year old man with PTC, hemosiderosis and hepatocarcinoma. The tumor was diagnosis by ultrasound in an imaging control that allows an opportune surgical treatment. The patient after 18 month to the surgery has no problem. Magnetic resonance value in diagnosis of the tumour and hemosiderosis are discussed.
